Transitioning from community Pharmacy to Hospital Pharmacy & GP Practice.
The aim of this initiative is to provide a balanced experience for community pharmacists that wish to gain hospital and GP Practice experience for improved clinical practice for patients within hospital/mental health settings. In addition, our trainee pharmacists will bring in their transferable skills to support Hospital pharmacy department & GP Practice.
The trainee pharmacists from the community are assets to Hospital departments and GP Practices, and they are able to support in these main areas :
- Drug history and Clinically screening of drug charts for safety and accuracy after they have been re-written by Prescribers.
- Ordering inpatient /TTAs items under the wards
- With the vast community experience including patient facing skills,they are helpful in advising other clinical staff on effective remedies for minor ailments and other health conditions normally seen in the community setting.
- Help with any audits currently being carried out in the trust
The community pharmacists will benefit from this hospital experience training because they will have the opportunity to experience and understand how pharmacists contribute to the care of patients both in the acute and General medical wards which will enhance their clinical skills.
This is a programme intended to modernise careers for community pharmacists, who may wish to consider hospital pharmacy as a career in the future. There are a number of Hospitals currently supporting this initiative.
- Practising GPhC registered Pharmacists.
- Each participant would have been practising for minimum of one year in a community pharmacy.
- Completed current Disclosure and Baring services check(DBS)
- Completed occupational health check declaration.

- These include opportunity to see Hospital Dispensary operations using JAC system.
- Awareness of Clozapine monitoring system in a mental health setting.

- TTO, outpatient and inpatient drug chart screening

- Gain experience in managing long-term conditions and medication reviews.
- Support holistic and continuous care for patients
- Work closely with GPs and healthcare teams.
- Apply community pharmacy skills to enhance GP services.
